The 3 D’s of Aging

Lunchtime lift adDyspigmentation, Deflation & Decent

The first ‘D’ of aging is dyspigmentation. Dyspigmentation is the increased production of skin pigmentation which often occurs in the aging process due to hormonal changes and sun damage. Various laser and light based treatments are used to treat surface pigmentation from the sun, whether it’s sun spots, broken capillaries, or mottled pigmentation. However, the hallmark of aging is deflation. People look older because they lose volume. Volume loss is particularly evident in the cheek and under eye area when the youthful contours of the cheek are lost. The perception of youth is a wide mid face, wide in the upper and tapering in the lower one third of the face.
Aging effects all levels of the face and changes are evident not only on the  skin  but also the fat pads, subcutaneous tissue, muscle, fascia and bone. Therefore all aspects of aging need to be addressed in a cosmetic procedure. Dermal fillers are most effective in the middle and lower third of the face. We now use a global approach to cosmetic rejuvenation. The upper one third of the face is treated with Botox to relax the muscles of facial expression. The middle third of the face is volumized and filled to replace volume, particularly the apex of the cheek create the perception of youth and finally the lower face is contoured and shaped. Dermal fillers are natural hyaluronic acid based fillers that are used to shape the jaw line, the prejowl hollows, chin, smokers lines as well as the area in front of the ears. Lips are enhanced in some women who loose volume as a result of aging. The last ‘D’ of aging is descent or sagging. Sagging is due to loss of collagen and elasticity of the skin. The youthful triangle with the wide mid face is reversed such that the base of the triangle is now at the lower one third of the face. Ultherapy is a novel technology that delivers focused ultrasound energy to the deep layers of the skin thereby resulting in the formation of new collagen which over a period of 3-6 months, lifts and tightens the skin. Can ultrasound lift skin? Multiple randomized controlled trials since 2006 have demonstrated the safety and efficacy of this procedure and to date 140,000 procedures have been performed worldwide for nonsurgical face, neck and brow lifts. The beauty of Ultherapy is that there is no downtime. It’s a lunch hour procedure and you can return to work the same day. Ultherapy is however not a face lift. Rather it’s more about slowing the clock than stopping the clock and works well for women between the ages of 35 – 60 who are showing mild to moderate laxity in their face and neck. Typically 2 treatments are recommended for best outcomes spaced 12-18 months apart.
